To make an appointment at the Des Moines Pastoral Counseling Center, please contact our intake coordinator, Carol Leonard by email, firstname.lastname@example.org or telephone, 515-274-4006, ext. 108 between the hours of 8 a.m. and 5 p.m., Monday through Friday.
Carol, or another trained staff member, will ask you several questions to determine how best to address your reasons for seeking professional counseling. She will also want to know what time of day you are able to come to the Center for counseling.
When an appointment has been scheduled, an email will be sent to you with a link to set up an account in our Patient Portal. Our initial forms can be accessed there, printed, filled out by you, and brought to your first appointment.
If a computer is not available to you, another option would be to come 20 minutes early to the first appointment and fill out the forms in our office.
It may be helpful for you to check with your insurance company before your appointment to be sure you know what your benefits are and whether or not you need any type of referral or pre-authorization. You may also wish to check with your insurance what their expectations are for a co-pay.
When you arrive for your appointment, you will need to have a form of identification (commonly a driver’s license) to be copied by our receptionist. We will also need a copy of an insurance card if you are using insurance to help pay for your counseling. Then you will meet with your counselor.
